Please note these appointments will be held at THE HOLLIES SURGERY on Elbow Lane. Please go there for your appointment, do not go to the Chapel Lane building.
This service is delivered as part of a Joint Working arrangement between GlaxoSmithKline UK Limited and Chapel Lane Surgery.
GlaxoSmithKline will not have access to your patient medical records or personal information and will have no involvement in your treatment decisions.
