History and Background

Formby Medical Group has been formed from the merger of Chapel Lane Surgery and The Hollies Family Surgery. Both practices have a long standing in the community. Chapel Lane Surgery first opened in Formby in the 1970s, originally located on Duke Street, before moving to a new building on Chapel Lane in the early 2000s. The Hollies first opened in the 1980s, with a modern extension built to an existing house, and is based on Elbow Lane. We will continue to use both sites for clinics for all patients.
